The CIS Care Coordination program assists participants and family/caregivers with navigation of the complex service system including health insurance, medical groups (CIS partners with Facey and Health Care Partners medical groups), doctors’ & specialists’ appointments, transportation to medical appointments, interaction with pharmacies and government-based service entities. We provide guidance on public benefits to low income, under-informed, under-served seniors of various cultures. Care Coordination services include assistance with Medi-Cal and Medicare applications, guidance in choosing a Medicare Advantage Plan and assisting our participants to respond to mail. CIS makes referrals for legal consultation to Bet Tzedek House of Justice.

Nutrition and Fitness Programs have support from a Medical Assistant/Care Coordinator with monitoring of special diets, vital signs, weight management program, and communication with health support systems. CIS partners with North Valley Pharmacy for medication management. The Pharmacist Assistant visits CIS once a week to deliver medications meet with participants as necessary for consults and may give second opinions on prescribed medications.  Most medication co-pays are waived for CIS participants who qualify and use North Valley Pharmacy.

CIS Care Coordination Services are enrichments in a social model day program which are not covered financially by our service contracts. This makes CIS a niche day program with "health care-light" services; we prevent some participants with modest medical or aging challenges from going to clinically-based Adult Day Health Care or more institutional nursing home services pre-maturely.
